Parent & Community Stakeholder Survey

Hello St. Anthony parents and community stakeholders,

On behalf of the school advisory board, Principal Karen Bogol is requesting your participation in completing a brief survey. Our goal is to have at least 80% of school families and as many parishioners as possible complete the survey prior to noon on Monday, October 3rd.
 
The purpose of this survey is to find out your opinions about St. Anthony de Padua Catholic School. The information gathered will then be used to make continuous improvements to our curriculum and faith formation. Read each statement and choose the response that most closely matches your opinion.
